Platelets, or thrombocytes, are tiny, but important cell fragments that form clots and stop or prevent bleeding. Platelets are used to treat patients who are currently bleeding due to injury or surgery, or who are at risk of bleeding due to lower-than-target platelet counts.

- The minimum yield specification of a standard apheresis platelet in the United States is 3.0 x 1011 platelets per unit. Standard single donor platelets normally contain an average 3.5–4.0 x 1011.
- Each unit’s total volume averages 305 ml and is suspended in platelet additive solution (65%) and plasma (35%).
- A very small proportion of the Red Cross platelets are low-yield (2.8 – 2.9 x 1011); these are used interchangeably with standard-yield platelets for all clinical indications.
- The anticoagulant used is acid citrate dextrose (ACD).
- All platelets that Red Cross provides are suspended in a platelet additive solution (PAS).
- PAS is a crystalloid, isotonic, saline nutrient media that replaces 65% of the plasma in which platelets are stored.
- The use of PAS has been shown to reduce febrile non-hemolytic transfusion reactions (FNHTRs) and allergic transfusion reactions (ATRs) by up to 75% (Cohn, C.S., et al. Adverse Reaction Rates for PAS C PLTs. Transfusion 2014, 54: 1927-1934.)
- The use of PAS platelets may also minimize the risk of other adverse reactions associated with incompatible plasma.
- Platelets are stored at room temperature (20-24°C) for up to 5 days.
- Platelets must be agitated to prevent them from clumping.
- CSP Platelets are stored at 1-6 C for up to 14 days.
- CSP do not require agitation.
- Scheduled orders are recommended for up to 70% of a hospital’s complete RBC needs to ensure a consistent supply.
- Ad hoc orders for platelets are placed in Connect.
- In most instances, ABO matching is not required for platelets considering the replacement of 65% of the plasma with PAS.
- HLA-matched platelets are recommended for patients demonstrating immune-mediated platelet refractoriness.
- Platelets are shipped at room temperature with up to four units per box.
- Pediatric platelets are available on scheduled order with 4 or 6 150mL empty bags attached.
- CSP are shipped 1-10 C with up to seven units per box
